What Is Acupuncture?

Acupuncture(針灸禪渣 zhēnjiǔ) is an age-old healing practice of traditional Chinese medicine in which thin needles are placed at specific points in the body. It's primarily used to relieve pain but also has been used to treat other conditions. More than 3 million Americans use acupuncture, but it is even more popular in other countries. In France, for example, one in five people has tried acupuncture.

How Acupuncture Works

Acupuncture seeks to release the flow of the body's vital energy or "qì氣 " by stimulating points along 14 energy pathways. Scientists say the needles cause the body to release pain -- and may boost blood flow and change brain activity. Skeptics say acupuncture works only because people believe it will, an effect called the placebo effect.

Does Acupuncture Hurt?

Acupuncture needles are very thin, and most people feel no pain or very little pain when they are inserted. They often say they feel energized or relaxed after the treatment. However, the needles can cause temporary soreness.

Effectiveness

There isn't yet a consensus in the West about whether acupuncture is effective. The various governing bodies, agencies, and medical societies have differing opinions. Medical surveys or clinical trials also produce differing results. There has been considerable controversy surrounding its value as a therapy and whether it is anything more than placebo. However, many people say that the treatments helped them or relieved chronic pain, and now the procedure has a level of acceptability.

第3點也是中醫界常見的觀點，表示對用西醫方法研究中醫的失望，甚至是擔憂及恐懼。其實，'隨機對照試驗'（RCT）研究方法不僅限于西醫，在社會學等其它研究中也有廣泛應用，是一種公認的科學研究方法。說此方法不適用中醫針灸研究為時尚早。

第4點是問題產生的關鍵。因為絕大多數西方針灸試驗的結果表明，針灸療法同不治療或常規治療比，是有效的，只是同安慰劑對照比，經常沒有顯著性差異。最常用的試驗對照是，'真刺假穴位'，即選不相關的體表位置針刺作為安慰劑對照。這種對照的問題是，人體有'泛穴'現象，針刺人體表面幾乎任何部位，都可能產生一定針灸效果（但不是說穴位無特異性），所以'真針假穴'對照組不是安慰劑，也是一種針灸治療；另外一種有時使用的安慰劑對照組是，'假刺真穴位，即用一種特殊設計的假針具，在真的穴位上' *** '，而患者不能識別真假。這種對照組的問題是，除了假刺也有皮膚接觸和輕度的 *** 外（不然不能蒙騙患者），患者接受的不僅僅是無安慰劑暗示，同時還接受了針灸附帶的心身療法（包括放松療法、環境療法、醫生指導、甚至音樂療法等），這些心身療法已經被證明治療疼痛等多種針灸適合癥有效，這些'有效療法'加強了安慰劑組的作用，淡化了治療組的效果，可用'氣球理論'來解釋。另外，'真刺假穴位'也有同樣的問題。因此，以上兩種安慰劑對照，都不是真正的安慰劑對照，而是不同療法的對照，所以需要很大的樣本才能比較出差別來。這就是為什么大多數'中小樣本'的試驗結果是針灸治療同安慰劑治療同樣有效。這個觀點可以得到Vickers 2012年發表在Annal Int Med 文章數據的支持。

第5點陳述了公認事實，可以解釋為什么在西方的針灸試驗結果同中國的很多報道不一致。因為近幾十年來西方流行的是'軟針灸'，而中醫傳統針灸主要是'硬針灸'。如上所述，'軟針灸'附帶的心身療法內容可能要大于'硬針灸'，所以西方針灸試驗同傳統中醫針灸有一定不可比性，需要深入研究。也就是說，即使'軟針灸'同安慰劑比較無差異，也不能證明傳統的'硬針灸'也無效。

第6點是說針灸療法（軟針灸和硬針灸）已經包含了心身療法和安慰劑效果作為一個'整體療法'（Holistic modality），這個整體是很難分割的。目前還沒有一種好的辦法可以用來驗證其中的每一個單一因素的效果，應該深入探索新的研究方法。從臨床實際看，驗證'整體療法'的效果，使用'實效性臨床試驗'（pragmatic trial）是比較現實的，最近，已經有一些臨床試驗結果支持這一觀點。
